All of our Registered Nurses are directly involved as Case Managers and with you and your physician develop a plan of care that assists you in meeting your goals.
Our Nurses coordinate any additional services you might need (Physical Therapy, Occupational Therapy, Speech Therapy, Social Work, or Home Health Aide Services) directly in your home and with the approval of your physician’s orders based on the needs of each individual.
